We are seeking a skilled and compassionate Support Worker to join our team at Chorus. As a Support Worker, you will play a vital role in supporting seniors and NDIS customers to remain in their homes, promoting their independence and well-being.
Key Responsibilities- Provide person-centered support to customers with varying needs, including domestic assistance and community access.
- Work collaboratively with customers, their families, carers, and other professionals to develop and implement tailored support plans.
- Conduct regular assessments and reviews to ensure customers' needs are being met and support plans are effective.
- Develop and maintain strong relationships with customers, their families, and carers to ensure a high level of customer satisfaction.
- Participate in ongoing training and professional development to enhance your skills and knowledge in support work.
- Certificate III Individual Support - Aged Care or Disability, or other relevant qualification.
- Experience working with seniors, people on the mental health recovery journey, and those living with disability.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups of people.
- Ability to work independently and as part of a team, with a strong focus on customer-centered care.
- Current Australian Drivers Licence with no restrictions, and a reliable and roadworthy motor vehicle for travel to customers' homes.
- NDIS Worker Screening Check (or willingness to apply for one prior to commencing employment).
- Current First Aid Certificate with 12 months or less currency (or willingness to apply).
- A competitive hourly rate from $32.20.
- Attractive salary packaging options (up to $15,900 per year tax-free) and an additional $2,650 for Meals and Entertainment.
