The Role

We are seeking a highly skilled and compassionate Support Worker Multi Skilled to join our team. As a Support Worker, you will play a vital role in supporting seniors and NDIS customers to remain in their homes. This role is based in Kwinana and surrounding areas, and you will work closely with customers to provide tailored person-centered services and activities of their choice.

Key Responsibilities
  • Provide domestic assistance services to customers, including household chores and personal care
  • Work collaboratively with customers, their families, carers, community agencies, and other professionals to ensure comprehensive support
  • Develop and implement person-centered plans to meet the unique needs and goals of each customer
  • Conduct regular assessments to identify changing needs and adapt support plans accordingly
  • Ensure the safety and well-being of customers, staff, and the community
Requirements
  • Certificate III Individual Support - Aged Care or Disability, or other relevant qualification
  • Experience working with seniors, people on the mental health recovery journey, and those living with disability
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Current Australian Drivers Licence with no restrictions
  • Reliable and roadworthy motor vehicle to travel to customers' homes and assist them to access the community
  • Comprehensive motor vehicle insurance and registration
  • NDIS Worker Screening Check (or willingness to apply for one prior to commencing employment)
  • Current First Aid Certificate with 12 months or less currency (or willingness to apply)
