About this role: We're seeking a passionate and empathetic professional to join our team as a Home-Based Care Support Worker. In this fulfilling position, you'll have the opportunity to make a meaningful impact in the lives of children and young people.

This is an exciting role that involves providing essential support to clients and their families within our Foster Care program. You'll work closely with our HBC Case Manager and Coordinator to ensure each child's and young person's unique needs are met, while prioritizing their emotional and physical well-being.

You will be responsible for planning engaging activities within the home, exploring the community, and assisting with transporting clients to appointments such as school, extracurricular activities, and family visits.
