Job Overview

We are seeking an experienced Clinical Partner to support our team in delivering high-quality, person-centered clinical care that supports clients to live well in their own home.

Key Responsibilities
  • Assess client care needs and develop, review, and implement care plans.
  • Deliver high-quality clinical care in partnership with clients and key stakeholders.
  • Collaborate with the care management team to achieve optimal client outcomes in the community setting.
  • Provide guidance to care staff to ensure safe and continuous delivery of care.
Essential Qualifications
  • Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
  • Passion for and previous experience in client-focused service delivery in aged and community/home care settings.
  • Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards.
  • Excellent communication and collaboration skills, with a commitment to advocacy and person-centered care.
Desirable Skills
  • Nursing experience in aged care and community care.
  • Ability to identify and implement innovations in care delivery and service improvement.
