Clinical Partner

The Clinical Partner is a key member of our multidisciplinary team, working in partnership with clients and their families to deliver high-quality, person-centered clinical care in the comfort of their own homes.

Key Responsibilities:
  • Conduct thorough assessments of clients' clinical care needs and develop, review and implement comprehensive care plans that meet their unique requirements.
  • Provide expert clinical care, working collaboratively with clients and their support networks to achieve optimal outcomes.
  • Collaborate closely with the care management team to ensure seamless coordination of services and achieve exceptional client results.
  • Offer guidance and support to care staff to ensure consistent delivery of high-quality care and promote a safe and supportive environment.
Essential Requirements:
  • Registered Nurse qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
  • Proven experience in delivering client-focused care in aged and community/home care settings, with a strong commitment to advocacy and person-centered care.
  • Knowledge of relevant legislation and regulatory frameworks, including the Aged and Home Care Reforms and Aged Care Quality Standards.
  • Excellent communication and interpersonal skills, with a proven ability to work effectively in a multidisciplinary team and independently across multiple sites.
