Clinical Care Partner

Join a leading organization in Aged and Dementia Care, Palliative Care, Rehabilitation, Mental Health services for older people, and other related Health and Aged Care services.

We embed evidence-based best practices in our services and provide these through Home Care, Sub-Acute Hospitals, and Residential Aged Care.

About the Role

The Clinical Partner plays a vital role in supporting the care management team to deliver high-quality, person-centered clinical care to clients in their own homes.

This is an exciting opportunity to make a real difference in the lives of our clients and their families.

Key Responsibilities:
  • Clinical Care Planning: Assessing clinical care needs and developing, reviewing, and implementing care plans that cater to the individual needs of our clients.
  • Clinical Care Delivery: Delivering evidence-based, best practice clinical care in partnership with clients and key stakeholders, ensuring optimal client outcomes in the community setting.
  • Team Collaboration: Collaborating with the care management team to achieve client-centered goals and objectives.
  • Staff Guidance: Providing guidance to care staff to ensure safe and continuous delivery of care.
Requirements:
  • Nursing Qualification: Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
  • Client-Focused Experience: Passion for and previous experience in client-focused service delivery in aged and community/home care settings.
  • Aged Care Reforms: Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards.
  • Communication Skills: Excellent communication and collaboration skills, with a commitment to advocacy and person-centered care.
  • Teamwork: Ability to work effectively in a multidisciplinary team and independently across multiple sites.
  • Professional Development: Commitment to professional development and ongoing learning.
