About the Position

We are seeking a skilled and compassionate professional to deliver client-centred care to elderly individuals and people living with disabilities in their homes.


Key Responsibilities:
  • Conduct comprehensive assessments using standardized tools and clinical observation.

  • Prepare clear and timely reports using structured templates and clinical tools.

  • Collaborate with multiple stakeholders, including families, support coordinators, and other health professionals to ensure holistic care.

Requirements:
