Job Title: Aged Care Specialist

Job Summary:

As an experienced Aged Care Specialist, you will play a vital role in conducting comprehensive assessments to identify the complex needs of clients. You will be responsible for collaborating with clients, families, and multidisciplinary teams to provide tailored support services under the Aged Care Act.

Daily Responsibilities:

  • Collaboration and Teamwork: Work closely with clients, families, and healthcare professionals to promote a supportive team culture and ensure seamless care delivery.
  • Aged Care Assessments: Conduct accurate and thorough aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
  • Review and Revision: Regularly review and revise support plans to meet changing support needs and ensure optimal outcomes.
  • Autonomy and Performance: Work autonomously and as part of a team to meet performance indicators within set timeframes, maintaining high standards of quality and efficiency.
  • Continuous Improvement: Stay up-to-date with funded and non-funded aged care services, contributing to the continuous improvement of assessment processes and outcomes.

Qualifications and Experience:

  • Hold a health-related tertiary qualification (e.g., Registered Nurse, Occupational Therapist, Physiotherapist, Social Worker).
  • Be currently registered with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Possess a valid Australian Driver's license and access to a registered, insured vehicle.
  • Have a minimum of one year of experience delivering aged care services in Australia or overseas, including to people living with dementia.
  • Be willing to apply for a Police Check.
