About the Role

The Central Adelaide Local Health Network is seeking a skilled and dedicated individual to fill the position of Transition Care Program Coordinator. As a key member of our team, you will play a vital role in supporting older individuals as they transition from hospital to community-based care.

Key Responsibilities

  • Liaise with hospitals to identify suitable patients for the Transition Care Program
  • Develop short-term care coordination plans to ensure smooth transitions to rehabilitation services
  • Facilitate ongoing case management in the community and maintain essential linkages with aged care agencies, primary health care, and General Practice

About You

We're looking for a dedicated professional with a strong background in Allied Health. If you meet the following criteria, we want to hear from you:

  • Qualifications: Degree in a relevant Allied Health discipline with registration eligibility
  • Communication Skills: Effective in problem solving, conflict resolution, and team collaboration
  • Experience: At least 3 years post-graduate experience in Allied Health, particularly with older adults
  • Holistic Approach: Ability to develop and implement community-focused therapeutic interventions
