Community Mental Health Nurse - Care Coordinator Role

The role of the Community Mental Health Nurse - Care Coordinator involves managing a caseload in accordance with CPA procedures and risk management protocols. This includes creating and continually updating person-centred care plans, building therapeutic relationships with clients, and assessing risk to design personalised risk management plans.

Key responsibilities include providing leadership for junior staff through supervision and appraisal, participating in the planning, development, and evaluation of multidisciplinary team services, and holding personal responsibility for defined projects and areas of responsibility. Additionally, you will mentor and supervise students on practice placements, carry out MDT activities, identify safeguarding concerns, and ensure compliance with the Mental Health Act 1983.

As a skilled professional, you will also be responsible for monitoring physical health, promoting physical wellbeing, supporting carer and service user involvement, and providing profession-specific interventions as required. Furthermore, you will participate in team training, mandatory training, and ongoing professional development.
