We are seeking a skilled Case Manager - Kinship Care to join our team in the Hume region. The successful candidate will have a demonstrated ability to effectively engage with children, young people, and their kinship families, and a good understanding of the inter-generational factors and complex relationships between extended family members that can impact kinship placements.

The Case Manager - Kinship Care will have a strong focus on outreach in the community, with service delivery across the Shires of Moira, Murrindindi, Strathbogie, Mitchell, and City of Greater Shepparton.

Key Responsibilities
  • Provide case management and casework services to young people and families in out-of-home care
  • Assess and develop kinship care plans in collaboration with kinship families and other stakeholders
  • Provide support and guidance to kinship families to ensure the best possible outcomes for children and young people
  • Develop and maintain relationships with community agencies and services to ensure comprehensive support for kinship families
Requirements
  • Previous experience in the provision of case management and casework services to young people and families in out-of-home care
  • A sound knowledge of the Children, Youth and Families Act 2005
  • A Bachelor or Diploma qualification in Social Work, Youth Work, or other related discipline
