Key Responsibilities- Provide case management and support to clients, utilizing case management frameworks and tools as per TSA policy and procedure.
- Maintain partnership arrangements and build service relations with other agencies.
- Ensure accurate and up-to-date client records and data are maintained in accordance with TSA standards.
- Participate in client feedback sessions and completion of outcome reports as required by the agency and funding bodies.
- Identify risks to clients, staff, and the organisation and report these to management as soon as possible.
- Tertiary qualification in social work, welfare, community development, or a related field.
- Relevant experience in a social service environment, preferably in the youth sector.
- A current and valid Working with Children's Check.
- A current and valid Victorian Driver's Licence.
