Job Title: Advanced Case Manager – Foster Care and Disability Support
Key Role SummaryThe Advanced Foster Care Case Manager plays a pivotal role in ensuring the safety, stability, and well-being of children and young people in foster care. As a senior team member, you will be responsible for managing complex cases, particularly those involving disability, and helping plan for long-term care and permanency.
This position involves close collaboration with carers, families, and other professionals to achieve the best outcomes for each child. You will also have the opportunity to mentor others, share expert advice, and contribute to improving service delivery.
A strong understanding of trauma, child development, disability, and the foster care system is essential for this role.
Key Responsibilities:
- Manage complex cases, especially those involving disability, and help plan for long-term care and permanency
- Work closely with carers, families, and other professionals to get the best outcomes for each child
- Mentor others, share expert advice, and help improve how services are delivered
To succeed in this role, you will need:
- Bachelor's degree in Social Work, Psychology, Social Sciences or related discipline with appropriate practical experience
- Certificates in Shared Lives Victoria and Step by Step Assessment Tool, highly regarded
- Current valid Victorian Driver's Licence
