About the Role

The coordinator will be responsible for ensuring accurate reflection of aged care resident care needs and ensuring East Grampians Health Service receives rightful remuneration for care provision.

They will ensure standardization of AN-ACC process and procedures, and take a role in educating aged care staff.

About You
  • Hold qualification as a Registered/Enrolled Nurse with AHPRA
  • Previous experience in completion of aged care assessments and documentation
  • Sound knowledge and understanding of relevant Aged Care Legislation, AN-ACC processes and Commonwealth Outcome Standards
  • Excellent communication, negotiation, organizational and time management skills
  • Ability to set priorities, work under pressure and complete tasks within specified time frames
  • Self-reliant, motivated, able to work independently and innovatively
  • Ability to liaise across full spectrum of services and relate positively to a wide range of people
