Senior Practitioner Role

This role is a key position within the Care Hub, providing support to children and young people entering care for the first time.

The Key Practitioner will be responsible for undertaking assessments, coordinating care plans, and serving as the primary point of contact for the child/ren and young people during their time in the hub.

They will also coordinate the multidisciplinary team in assessing, planning, and supporting children and young people in the hub.

Key Responsibilities:
  • Managing risk and wellbeing, working within the Best Interest Framework.
  • Understanding how trauma affects the development of young people.
  • Solid understanding of the Child Protection and Welfare system in Victoria.
  • Coordination and facilitation of care team meetings and professional meetings with young people, families, and other agencies.
  • Coordinating a multidisciplinary team in assessment, planning, and support to children and families.
  • Development of plans and assessments, report writing, case noting, recording, and monitoring compliance.
  • Monthly statistics reporting.

  • Negotiable through increments dependent upon experience.
