Social Worker – Young People Support Specialist

This role is an opportunity to make a positive impact on the lives of young people in care. As a Social Worker, you will work closely with children aged 14 and above, supporting them as they prepare for adulthood and independence.
You will assess their needs, develop tailored care plans, and coordinate services to support their education, health, and emotional well-being. This includes creating Children Looked After (CLA) Plans, Education, Health and Care (EHC) Plans, and transition plans.
Responsibilities:- Assess the needs of children in care and develop tailored care plans
- Coordinate services to support the education, health, and emotional well-being of children in care
- Work closely with families, carers, and partner agencies to ensure consistent support
