Job Opportunity: Support Young People in Transition
Our organisation provides innovative solutions for young people leaving care. Our case managers work closely with young people to stabilise their housing, build independent living skills and set them up for a better future.
Key Responsibilities Include:
- Direct casework with young people to support their transition to independent living.
- Case management responsibilities.
- Relating innovative solutions in a new and developing program.
- Building and working in partnership with other organisations across the state.
- Assessment and planning.
The Skills You Will Need:
- A relevant tertiary qualification in social work, psychology, welfare or related discipline and experience.
- A trauma-informed practice lens.
- A person-centred approach, which supports the young person to make and be responsible for their own informed life choices.
- Tailoring support to the needs of the young person.
- The ability to assist young people to transition from care to independence through the establishment of safe, stable, and secure housing and support.
