Care Manager Professional

As a Clinical Leader in Aged Care, you will play a pivotal role in delivering high-quality and person-centered care to residents. You will oversee the development and implementation of individual care plans, ensuring the specific medical, physical, and emotional needs of each resident are addressed.
About the Role:- Resident Care: Ensure the delivery of compassionate and competent care to residents.
- Staff Supervision: Manage and lead a team of healthcare professionals, including nurses, caregivers, and support staff.
- Care Planning: Oversee the development and implementation of individual care plans for each resident.
- Medication Management: Ensure proper administration and management of medications for residents.
- Family Communication: Communicate effectively with residents' families or legal guardians, providing updates on their loved one's condition.
- AHPRA Registration.
- Minimum 2 years' experience in Aged Care as a Registered Nurse.
- Knowledge of Aged Care accreditation standards.
- Comprehensive knowledge and experience with ANNAC.
- Expertise in medication administration and wound care with a commitment to ongoing clinical excellence.
- Strong leadership, communication, and interpersonal skills.
- An outstanding proficiency in care planning, goal setting, and evaluation documentation.
- Capability to effectively lead and manage a team.
