Hearth Support Services is a dynamic NDIS Registered Service Provider seeking a skilled Relationship Manager to join our team at our Glen Iris office.

The Relationship Manager role is crucial in establishing and maintaining relationships between participants, their family members, support workers, and other stakeholders. This position requires strong communication and interpersonal skills to ensure seamless coordination and support.

Responsibilities:

Main Responsibilities
  • Develop and maintain relationships between participants and their team of Hearth Support Workers
  • Evaluate participant needs and develop Hearth's support programme
  • Monitor participant progress and prepare submissions for NDIS plan reviews as required
  • Maintain relationships with support coordinators, house supervisors, and other external stakeholders
  • Coordinate staff rosters for each participant and resolve rostering issues proactively
  • Accurately record case notes and conduct client reviews that reflect forward planning in line with the participant's NDIS plan
  • Ensure compliance with Hearth policies and procedures to guarantee a safe working environment and timely incident reporting

Requirements:

Key Requirements
  • A minimum of 2 years' experience as a Case Manager/Support Coordinator in community services, welfare support, disability support, or social work (desired)
  • An understanding of the NDIS and the role of a Disability Support Worker
  • Strong administrative and communication skills, including case noting and report writing
  • Excellent time management and organisational skills
  • High levels of resilience and strong interpersonal skills
  • A working knowledge of Microsoft Office suite and employee rostering systems
  • A driver's licence and own vehicle

Estimated Salary: $65,000 - $80,000 per annum, depending on experience.
