Job Description

About the Role: We are seeking a skilled Care Coordinator to join our team at Signature Care Operations in Wagga Wagga NSW.

Job Overview:

Responsibilities:

  • Determine individualised care plans that cater to the unique needs of our residents.
  • Communicate effectively with residents, families, and other healthcare professionals to ensure seamless care delivery.
  • Maintain accurate and up-to-date records of resident care plans and progress.
  • Collaborate with the multidisciplinary team to deliver high-quality care services.

Requirements:

  • A current unrestricted registration with AHPRA as a Registered Nurse.
  • Ability to assess, plan, and implement care that addresses the physical, social, and psychological needs of residents.
  • Excellent communication and clinical problem-solving skills to navigate complex care situations.
  • National Police Check less than 12 months old and three COVID-19 vaccinations.
