About the Role:

The primary responsibility of the Community Care Support Specialist is to deliver a service which focuses on assisting elderly residents to remain as independent as possible by connecting them with care and support services.

This includes registering care applications, managing client intake process and enquiry line, determining client needs, arranging for assessments and funding approvals, connecting clients with care providers, records management, and travel to our villages to conduct face-to-face assessments and build relationships with residents.

About You:

We are seeking an experienced professional with aged care assessment experience as a Regional Assessor or ACAT Assessor looking for a change from the hospital system and wanting to transition to an over-the-phone support role.

The ideal applicant will have current AHPRA Registration, practical experience working in the health and/or aged care industries, excellent care assessment and planning experience, strong leadership and people management skills, positive and compassionate nature, great customer service skills teamed with a positive attitude and a strong attention to detail, and ability to travel for up to a week at a time.
