Job Description:


  • Manage a case load of clients under CHSP/HCP approved funding.
  • Maintain budgets and manage care requirements to support client independence.
  • Onboard growth of new clients and coordinate with stakeholders.
  • Time management.
  • Understanding the Aged Care Standards.
  • Liaise with stakeholders and internal networks.

 

Required Skills and Qualifications:


  • Minimum 1 year experience in Case Management.
  • Relevant tertiary qualification in the community sector.
  • Experience in aged/community/disability sector, particularly with clients with complex needs.
  • Full Driver's Licence and own reliable registered and insured vehicle.
  • Smart Phone.
  • Knowledge of My Aged Care portal/assessment process.
  • Willingness to work in a team environment.
  • Desire to join a team committed to the care of others in a mission-based organisation.
  • NDIS clearance (or willingness to obtain one).
  • Police clearance (within the last 6 months or willingness to obtain one).
