Job Summary

We are seeking a highly skilled Community Outreach Specialist to join our team in Caboolture, Queensland. As a key member of our community services team, you will work closely with clients to assess their needs, develop personalized plans, and connect them with essential resources.

Key Responsibilities
  • Assess client needs and develop comprehensive case management plans
  • Provide ongoing support and guidance to clients in achieving their goals
  • Build and maintain effective relationships with community partners and stakeholders
  • Maintain accurate and up-to-date records of client interactions and progress
Requirements

To be successful in this role, you will require:

  • Tertiary qualification in Community Services, Social Work, or a related field
  • Relevant experience in a social service environment, including case management and client assessment
  • A current Queensland Driver's License and a valid Working with Children Check
