About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Hearth Support Services. As a Support Worker, you will play a vital role in supporting our participants in achieving their goals and living their best life.

The successful candidate will have experience supporting individuals with disabilities, particularly those with mental health conditions and complex behaviors. You will be responsible for providing personal care, assistance with community access and recreational activities, and transportation. Additionally, you will be required to maintain accurate records and communicate effectively with participants, families, and other stakeholders.

Key Responsibilities
  • Provide person-centered support to participants with disabilities, focusing on their individual needs and goals
  • Assist with personal care, including showering, toileting, and personal hygiene
  • Support participants with community access and recreational activities, including transportation
  • Maintain accurate records and communicate effectively with participants, families, and other stakeholders
Requirements
  • Professional or lived experience supporting individuals with disabilities, particularly those with mental health conditions and complex behaviors
  • Experience working with Hoists, PEG Feeding, and Epilepsy Management
  • Excellent communication and interpersonal skills
  • A valid driver's license and a reliable vehicle
  • NDIS Worker Screening Check
  • The legal right to work in Australia
