Comprehensive Mental Health Support for Children

As a Care Navigator, you will play a pivotal role in providing comprehensive mental health support to children under 12 at Central Coast Local Health District. Our vision is to deliver trusted care that improves the overall well-being of our community.

The Medicare Mental Health Kids Hub offers multidisciplinary care that identifies and addresses mental health challenges in children at an early stage, providing tailored support, treatment, and intervention based on evidence and validated assessment tools.

Key Responsibilities:

  • Leading inter and multidisciplinary care planning, acting as an information conduit within CCLHD and with external partners.
  • Enhancing knowledge, skills, and confidence in working with neurodiversity and mental illness, and understanding NDIS and Early Childhood Early Interventions (ECEI) processes via multi-disciplinary and multi-agency teams.
  • Collaborating with families to create and implement care plans, complete referrals, and liaise with schools and agencies.
  • Assisting in developing systems, processes, and tools for consistent engagement, referral, and transition, including managing an allied health service map.
  • Coordinating MDT meetings, collating client information, and aligning goals between families and MDT.

As part of a team consisting of medical professionals, allied health staff, psychologists, and early childhood development experts, you will work collaboratively to deliver bio-psycho-socio-cultural assessments and interventions that complement and enhance existing services for children and their families.
