We are seeking a skilled Wellbeing and Health In-reach Nurse to join our team. The ideal candidate will have strong clinical nursing skills, experience working with children and young people, and the ability to provide complex nursing care in a school setting.
- This is an exciting opportunity to make a difference in the lives of our community's youth and their families.
The successful applicant will be responsible for facilitating identification of health, wellbeing and social needs and coordinating appropriate early intervention, assessments, and referral to services for school aged children and young people attending designated primary and secondary schools.
You will work in identified local schools and apply your specialist clinical nursing knowledge, experience and skill in providing complex nursing care for school aged children and young people in these schools.
About the RoleThis role requires the following key responsibilities:
- Facilitating identification of health, wellbeing and social needs
- Coordinating appropriate early intervention, assessments, and referral to services
- Providing complex nursing care for school aged children and young people
The successful applicant will also need to possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.
