Lead Care Professional

Our goal is to ensure residents receive high-quality, personalized care and support.

We are seeking a skilled Care Manager to lead our care team in delivering exceptional resident-centered care. This role is responsible for ensuring the effective delivery of care and clinical services to our residents.

Key responsibilities include:

  • Leading the care team to deliver timely and compassionate care to residents
  • Generating care plans and conducting clinical assessments
  • Monitoring medication management and developing AN-AAC submissions
  • Ensuring accurate recording of information in accordance with organisational and legislative requirements
  • Managing and monitoring comments and complaints in collaboration with the Facility Manager

This role requires strong leadership and communication skills, as well as the ability to build rapport and maintain professional relationships with residents and colleagues. A commitment to resident wellbeing and a willingness to work collaboratively to meet the needs of residents and the facility are essential.

Requirements

To be successful in this role, you will need:

  • Valid AHPRA Registration
  • Experience in preparing and conducting clinical assessments, care plans, AN-AAC and accreditation compliance and documentation
  • A valid NDIS Workers Screening Clearance (or the ability to obtain one)
  • An immunisation record of COVID-19 Vaccine
