Infinite Aged Care seeks a dedicated professional to provide high-quality clinical care to residents. The successful candidate will be committed to a full-time role, available for Monday to Friday 14:00-22:00 shifts.
This exciting opportunity involves working in our Wyoming NSW Clinical Care team as a Clinical Care Coordinator & Nurse Mentor. The key purpose of this role is to ensure that all residents receive the highest quality of clinical care.
- Provide clinical expertise and leadership of clinical and care service teams across the facility.
- Assist Care Manager to lead carer teams to meet all clinical needs of residents – including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
- Manage all aspects of the assessment, documentation and submission of AN-ACC requirements within the facility.
- Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
- Coordinate assessment, care planning and review processes within the facility.
- Support the admission of new residents to facilities for optimal care assessments and initial consultations with resident and families.
- Evaluate accurate information recorded in accordance with all organisational and legislative requirements (AN-ACC, resident records and staff time recording).
- Input and oversee into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents.
- Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility.
A minimum of 1 year experience as a Care Coordinator or similar leadership position is required. Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA) certification is also necessary.
- Post graduate qualifications in aged care are advantageous but not essential.
- Extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC and accreditation compliance and documentation.
- An engaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
- A high level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
- A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
- Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
- A valid NDIS Workers Screening Clearance (or ability to apply via NDIS).
