We are seeking a dedicated Care Coordinator to support our Home Care Packages clients.

Care Coordinators will work closely with clients, their families and carers to deliver high-quality care and services that meet individual needs and preferences.

The successful candidate will have recent experience in Aged Care, strong communication and interpersonal skills, and the ability to work independently in an office-based environment.

Key responsibilities include:

  • Developing and implementing comprehensive care plans that meet individual client needs
  • Providing ongoing support and coordination of services for clients
  • Collaborating with other health professionals and community agencies to ensure seamless care delivery
