Identifying the needs of older adults and connecting them with services that enhance their quality of life is a critical role in aged care.
The comprehensive assessment process involves understanding the individual's physical, emotional, and social needs. This information is used to develop a tailored plan that supports independent living and maximizes function and capability.
Families and carers play a significant role in this process, and effective communication is essential for ensuring the best possible outcomes.
Key Responsibilities:- Conduct thorough assessments to identify the needs of older adults.
- Facilitate referrals to services that support independent living.
- Promote the retention and maximization of function and capability in daily living.
- Work closely with families and carers, recognizing them as key stakeholders.
- Ensure all assessments are conducted in line with Department of Health contractual requirements.
- A minimum of one year of experience in aged care, including client assessments and/or service delivery.
- Tertiary education in a health-related discipline.
- Proficiency in Microsoft Office and client management systems.
- A current Australian driver's licence and comprehensively insured vehicle for travel within the service area.
