Support Coordinator Job Description

This position is responsible for delivering exceptional support to individuals with complex needs and high-level behavioural issues across various Supported Independent Living Houses.

We take a person-centred approach to care, focusing on providing human rights-based services that empower our participants to achieve their goals.

Key Responsibilities:
  • Ensure accurate and timely participant shift reporting and logging into the CTARS system.
  • Monitor incident reports, conduct investigations, and maintain alignment with NDIS standards and regulatory requirements.
  • Perform regular audits of the CTARS system to ensure compliance with data entry and reporting requirements.
  • Develop and generate customised reports and progress updates for employees, house coordinators, and management.
  • Maintain secure participant profiles and document management systems, tracking progress and sharing information as needed.
Requirements:
  • Excellent communication and interpersonal skills, with experience in the disability sector highly valued.
  • Ability to draw on information from various sources, use judgement to analyse, and make informed recommendations.
  • Proven project management skills, with a focus on achieving results and meeting deadlines.
  • A genuine passion for supporting people with disabilities and promoting inclusion.
  • Strong understanding of Microsoft Office, including Excel and SharePoint.
