Job Overview

We are seeking an experienced Case Manager to undertake child protection assessments, create individualised support plans, and provide intensive home visiting services to families.

About the Role

This position is responsible for identifying strengths and needs of a family, creating and implementing support plans to address specific risk factors, and providing regular home visits to monitor progress.

Duties and Responsibilities
  • Conduct thorough child protection assessments to identify areas of concern
  • Create and implement evidence-based support plans tailored to each family's needs
  • Provide regular home visits to ensure families receive necessary support and guidance
  • Maintain accurate records of activities and outcomes to inform future practice standards
Key Skills and Qualifications

To be successful in this role you will require:

  • A degree in social work or a related field
  • Substantial experience in case management, particularly in child protection
  • Strong understanding of child protection issues, including substance abuse, mental health, domestic violence and trauma's impact on a child's development
  • Familiarity with evidence-based practices and intervention models
  • Excellent communication and interpersonal skills
  • Ability to build relationships with stakeholders and navigate complex systems
