We are seeking a skilled Community Case Manager to support elderly individuals in living independently and with dignity in their own homes.
As a Community Case Manager, your primary responsibility will be to assess the needs of community-based elderly customers with Home Care Packages, develop personalized care plans, and manage budgets to ensure each customer's unique requirements are met.
Your role will involve collaborating and coordinating with Nurses and Allied Health professionals to deliver comprehensive care, while maintaining a person-centered approach.
You will work autonomously from home and within the community, with customers across Sutherland and surrounding suburbs.
About You- Previous experience in community-based case management is preferred.
- Experience with Home Care Packages is desirable.
- Current driver's license and reliable vehicle with insurance.
- Strong communication skills, both verbal and written.
- Demonstrated empathy and compassion.
- Excellent problem-solving abilities and critical thinking skills.
- Good organizational and time management skills.
- Cultural competence and ability to work with diverse populations.
- Advocacy skills to ensure clients' needs are met.
- Collaboration and teamwork skills to work effectively with interdisciplinary teams.
