About the Role

We are seeking a dedicated professional to join our team as a Transition Care Program Coordinator. As a key member of our team, you will play a vital role in supporting older individuals as they transition from hospital to community-based care.

Key Responsibilities
  • Liaise with hospitals to identify suitable patients for the Transition Care Program
  • Develop short-term care coordination plans to ensure smooth transitions to rehabilitation services
  • Facilitate ongoing case management in the community and maintain essential linkages with aged care agencies, primary health care, and General Practice
  • Mentor and guide less experienced staff to ensure high-quality care and practice standards
About You

We are looking for a dedicated professional with a strong background in Allied Health. To be successful in this role, you will need:

  • A degree in a relevant Allied Health discipline with registration eligibility
  • Effective communication and problem-solving skills, with the ability to work collaboratively as part of a team
  • At least 3 years of post-graduate experience in Allied Health, particularly with older adults
  • A holistic approach to care, with the ability to develop and implement community-focused therapeutic interventions
