As a leader in the delivery and coordination of Aftercare services, you will play a pivotal role in supporting consistent, high-quality care for individuals in need. This is an opportunity to lead meaningful change and improve outcomes following a suicidal crisis.
Key Responsibilities- You will coordinate stakeholder engagement and service integration to ensure that care is consistently delivered across the district.
- Support referral pathways and service improvements to embed best practice approaches in a multidisciplinary setting.
- Contribute to data reporting, evaluation, and quality initiatives to promote consistency in referral practices across the district or region.
- Promote awareness and understanding of Aftercare services across the district to enhance service visibility.
The Aftercare Lead provides leadership for the integration of aftercare services including The Way Back NSW Aftercare Service. The vision for this role is Leading Care, Healthier Communities. You will coordinate Aftercare services with relevant hospital and community mental health and suicide prevention services to ensure access to aftercare support for all who need it.
This position offers comprehensive community-based mental health care across South Western Sydney, catering to different age groups and providing specialized care throughout various life stages. The focus on community health and wellbeing makes it an ideal place to work, providing opportunities for professional growth and making a meaningful impact on the community.
Requirements- Relevant tertiary qualifications or equivalent experience in mental health and suicide prevention.
- Description of experience working collaboratively with diverse stakeholders to implement or enhance mental health services.
- Examples of how you've supported service design and implementation and contributed to service evaluation or data reporting in a mental health context.
- Australian C-Class driver's license (including P1 or P2).
