- About the Role:
- The purpose of this position is to provide person-centred care to residential and home-based aged care clients.
- You will work collaboratively with the multidisciplinary team to achieve holistic outcomes for our clients.
- Daily activities include providing personal care, completing documentation, and assisting with assessments and reviews.
Key Responsibilities:
- Provide high-quality person-centred care support to consumers, working collaboratively with stakeholders to deliver holistic outcomes.
- Work within a multidisciplinary team to achieve best care outcomes.
- Complete all documentation accurately and timely, and assist with assessments and reviews as necessary.
- Demonstrate consideration, understanding, and respect for clients and their families at all times.
Required Skills and Qualifications:
- Certificate III or IV in Individual Support (or equivalent).
- Experience in an Extended Care Assistant or Personal Care Worker role.
- Ability to work across all shifts.
- A current Police Check (not more than 12 months old).
