Job Overview

Key Responsibilities:

  1. Assess patient needs through comprehensive nursing assessments, identifying opportunities for care coordination and case management.
  2. Develop and implement personalized care plans that address physical, emotional, and social needs, ensuring alignment with hospital standards and professional guidelines.
  3. Collaborate with multidisciplinary teams to facilitate effective care transitions, minimizing delays and errors.
  4. Provide direct patient care, utilizing current nursing theory and research to inform practice and promote best possible outcomes.
  5. Participate in quality improvement initiatives, contributing to data-driven decision making and process enhancements.

Education and Training Requirements:

  • You must hold a valid nursing license and have current Basic Life Support certification recognized by the Heart and Stroke Foundation.
  • Prior experience in hemodialysis or medical/surgical settings is desirable, as is proficiency in French.
  • CNA certification in Nephrology is an asset, and successful completion of our Renal Orientation program within 2 months of entry is mandatory.
