Job Title: Case Manager for Bundaberg, QLD

We are seeking a skilled Case Manager to join our team at Conexson. As a Case Manager, you will play a vital role in assisting individuals in finding the best care options to meet their mental, physical, and emotional health needs.

Key Responsibilities:
  • Assess client needs and goals to develop a comprehensive care plan.
  • Plan and implement a care plan that ensures effective delivery of high-quality care and service.
  • Collaborate with other team members, organizations, and service agencies to ensure the best outcome for clients.
  • Manage feedback and complaints, and undertake continuous improvement to enhance service delivery.
  • Conduct and assist with internal audits to ensure compliance with industry standards.
  • Ensure confidentiality and maintain professional boundaries with clients.
  • Respond to problematic situations promptly and develop solutions to discuss with Coordinators and Managers.
Requirements:
  • Qualifications in Community Services, Human Services, Health Sector Management, Aged Care, and/or Clinical background (minimum Cert III) and a minimum of 3 years experience in the industry.
  • Demonstrated understanding of the Aged Care reform, Aged Care standards, the sector, and people who are aging in the community.
  • Experience in case management, preferably within the home and community care sector.
  • Experience in service development and driving continuous quality improvement and practice and service excellence aligning to Aged Care Quality Standards.
  • Highly developed interpersonal, communication skills, and emotional intelligence with the ability to foster relationships and connect with internal and external stakeholders.
Special Requirements:
  • Driver's License and National Criminal History Check.
  • Prescribed doses of COVID and Influenza vaccinations.
