
Aged Care Specialist Support Coordinator

The primary objective of this position is to provide high-quality support services to older individuals, enabling them to maintain their independence and live in their communities for as long as possible.
As a Case Manager, you will be an integral part of our Aged Care Team, working collaboratively to develop and review care plans that cater to the diverse needs of our consumers.
You will operate in accordance with the Aged Care standards, organisational requirements, and program guidelines to deliver exceptional service outcomes.
Key Responsibilities- Develop and review comprehensive care plans that align with the needs and goals of consumers
- Conduct thorough assessments to identify areas of need and implement tailored solutions
- Collaborate with interdisciplinary teams to ensure seamless service delivery
- Communicate effectively with consumers, families, and stakeholders to promote positive outcomes
- Bachelor's degree in Nursing, Allied Health, or Social Work, or equivalent experience
- Demonstrated knowledge of Aged Care reforms, legislation, compliance requirements, quality standards, and relevant policies
- Proven ability to work respectfully and collaboratively with Aboriginal and Torres Strait Islander peoples, their families, and communities
- Excellent communication, interpersonal, and problem-solving skills
