Role Overview

Main Responsibilities:
  • Conduct initial phone assessments, care planning, and goal setting for clients
  • Coordinate in-home services to support clients receiving care at home
  • Build and maintain effective relationships with various stakeholders
  • Monitor patient progress and update care plans as necessary
  • Liaise with healthcare professionals and specialists
  • Discharge planning, including referrals to long-term services
  • Provide telephone support, education, and advice
Key Skills and Qualifications:
  • Registered Nurse with unrestricted AHPRA registration
  • Recent experience in care coordination (highly desirable)
  • Strong customer service orientation and relationship management skills
  • Excellent clinical reasoning skills
  • Ability to work in a fast-paced environment
  • Proficient computer skills for data entry into patient electronic medical records
