Care Service Coordinator Job Description

We are seeking a highly skilled and experienced Care Manager to coordinate and manage care services in our residential aged care home. As a Care Service Coordinator, you will be responsible for planning, assigning, and overseeing duties to ensure that care is delivered in accordance with established policies and procedures.

Key Responsibilities

  • Use your clinical expertise to drive clinical excellence and improve the quality of care provided to residents.
  • Provide strong leadership in a resident-focused care environment.
  • Contribute to building a workplace culture that embraces innovation and collaboration.

Requirements

  • Successful completion of a minimum Bachelor Degree in Nursing (RN).
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Minimum 3 years experience in residential aged care.
  • Working knowledge of the requirements of the Quality Agency Standards, Specified Care and Service Principles of the Aged Care Act, Documentation Standards, Residents Rights, Aged Care Funding Instrument and other relevant legislation, regulations and codes of practices.
