Social Worker Grade 1 Position

An exciting opportunity exists for a new graduate social worker to join our team at St Vincent's Hospital Melbourne (SVHM). To be successful in this role, you will have completed a hospital-based placement and be passionate about delivering high-quality social work services to vulnerable populations.
The position will be based on the acute wards at SVHM, where you will work closely with patients, their families, and carers to co-ordinate, facilitate, and communicate patient treatment and outcomes. You will also have the opportunity to participate in quality improvement and research activities, as well as provide supervision and participate in clinical education programs.
Key Responsibilities- Undertake multi-dimensional, comprehensive psychosocial assessments and develop complex intervention and discharge plans
- Liaise with patients, their families, and carers to co-ordinate and facilitate patient treatment and outcomes
- Actively participate in quality improvement and research activities
- Provide supervision and participate in clinical education programs
- Promote and develop positive relationships within the department, wider organisation, and with community partners
- Bachelor Degree or Qualifying Masters in Social Work with graduation in the last 12 months
- Eligibility for membership of the Australian Association of Social Workers
- Social work experience in a hospital inpatient care setting is essential
- Current National criminal history check, or willing to obtain
