Complex Social Work Specialist

Monash Health is seeking experienced Grade 2 Social Workers to join our growing team at the Kingston Centre, working in a dynamic subacute hospital setting.

You will have the opportunity to work with a collaborative social work and multi-disciplinary team where you will play a pivotal role in contributing to a positive team culture.

The key responsibilities of this role include completing complex psychosocial assessments, developing patient-centred care plans and managing complex discharges. This position requires excellent written and verbal communication skills, as well as experience working in a community health or hospital setting.

Essential Qualifications:

  • Bachelor of Social Work or Master of Social Work (Qualifying)
  • Current driver's licence
  • Current Working with Children's Check
  • Eligibility for AASW membership
