Job Overview:

We are seeking a skilled Social Worker to join our team in a dynamic subacute hospital setting.

The successful candidate will work collaboratively with multidisciplinary teams to deliver high-quality patient care, contributing to a positive team culture and providing exceptional support to patients and their families.

  • Main Responsibilities:
  • Complete complex psychosocial assessments for patients
  • Develop patient-centred care plans and manage complex discharges
  • Work collaboratively with multidisciplinary teams to deliver high-quality care

Requirements:

  • Demonstrated experience as a social worker with excellent written and verbal communication skills
  • Bachelor of Social Work or Master of Social Work (Qualifying)
  • Experience working in community health or hospital settings
  • Current driver's licence, Working with Children's Check, and eligibility for AASW membership
