Family Support Case Manager

We are seeking a highly skilled and dedicated Family Support Case Manager to join our team at Findmea. As a key member of our team, you will play a vital role in supporting vulnerable families with children and young people, ensuring their safety, wellbeing, and stability.
The successful candidate will have a strong background in social work, psychology, or a related field, with experience in case management practice, risk assessment, and direct practice of family and parenting interventions. You will be able to work collaboratively with a multidisciplinary team to deliver a high-quality confidential service that conforms to legislative frameworks and ensures the safety and wellbeing of all families.
Key Responsibilities- Accept appropriate referrals and undertake initial assessments of families referred through The Orange Door.
- Undertake appropriate recording of all assessments, ongoing contact, reviews, and closures utilising the systems provided.
- Identify real or potential health, development, and family related dysfunction, referring on to appropriate services or taking appropriate intervention measures to assist parents to access services.
- Provide a Family Support service that meets the current DFFH program and service plan and implementation guidelines.
- Research, develop, implement, and evaluate group programs for both parents and children.
- Degree Qualifications: Degree qualifications in Social Work, Psychology, Behavioural Sciences, or a related field relevant to child and family welfare.
- Relevant Experience and Knowledge:
- Case Management Practice: Demonstrated knowledge and experience in Case Management practice including Risk Assessment and direct practice of family and parenting interventions.
- Legislation and Frameworks: Demonstrated knowledge of relevant legislations and frameworks relating to Child Safety, Risk Assessments, and Planning.
- Program Development: Demonstrated experience in the development, implementation, and evaluation of group programs for children, young people, and parents.
- Community Liaison: Demonstrated ability to liaise and work cooperatively with a wide range of Community organisations, service providers, and other relevant bodies, supporting vulnerable families within a culturally diverse community.
- Time Management: Demonstrated ability to manage time, prioritise and organise own workload whilst working within a multidisciplinary team.
- Interpersonal and Communication Skills: Excellent interpersonal and communication skills, including demonstrated ability to record appropriate case notes and produce reports supporting evidence-based cases.
- Computer Skills: Demonstrated intermediate computer skills including sound knowledge of the suite of Microsoft Office products and EDRMS such as Objective.
- Working with Children's Check (valid)
- Victoria Driver's License (valid)
- National Police Check (with Stat Dec) issued within the last 6 months
- Ability to commit to rosters with a proven track record of good attendance and punctuality.
