About the Role

We are seeking a dedicated and compassionate Case Manager to join our team at The Salvation Army. As a key member of our Youth Services team, you will be responsible for developing and implementing individualized case management plans that support young people in achieving their goals and improving their overall well-being.

Key responsibilities will include:

  • Developing and reviewing case plans to ensure they are tailored to each young person's needs and goals.
  • Scheduling regular reviews to monitor progress and make adjustments as needed.
  • Identifying and addressing physical, mental, emotional, spiritual, and social needs to promote overall well-being.
  • Maintaining accurate and up-to-date service documentation and records.
  • Collaborating with other organizations and agencies to provide comprehensive support to young people.

Requirements

To be successful in this role, you will need:

  • A minimum of a Diploma-level qualification in a relevant field, such as Youth Work or Community Services.
  • Current ACT Driver's License and Senior First Aid Certification.
  • ACT Working with Vulnerable People Certification.
  • Experience working with young people, particularly those impacted by homelessness and substance misuse.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
