Aged Care Specialist

As a skilled professional in the field of aged care, you will play a vital role in identifying the needs of older adults and connecting them with services that promote their independence.
Your Key Responsibilities:- You will conduct thorough assessments to identify the needs of older adults, providing them with tailored solutions to improve their quality of life.
- You will facilitate referrals to essential services that support independent living, ensuring that clients receive the best possible care.
- You will work closely with families and carers, recognizing them as key stakeholders in the care process.
- You will ensure all assessments are conducted in line with relevant contractual requirements, upholding the highest standards of professionalism.
- At least 1 year of experience in aged care, including client assessments and/or service delivery.
- Tertiary education in a health-related discipline, such as nursing, occupational therapy, physiotherapy, or social work.
- Current unrestricted registration with a relevant professional association.
- Proficiency in Word, Excel, Outlook, and client management systems.
- A valid Australian driver's licence and comprehensive vehicle insurance.
