Job Overview
The Huntington's Disease Service is a specialist team dedicated to supporting individuals and families affected by Huntington's Disease. Our team works closely with medical professionals, allied health, and community services to deliver integrated, evidence-based care.
Job Role
- This role involves delivering specialized case management and clinical support to individuals and families impacted by Huntington's Disease.
- You will coordinate care across community, inpatient, and outpatient settings, provide education and therapeutic interventions, and collaborate with health professionals and support services to ensure holistic, person-centred care.
Key Responsibilities
- Deliver high-quality, patient-centered care to individuals and families affected by Huntington's Disease.
- Coordinate care across multiple settings, including community, inpatient, and outpatient environments.
- Provide education and therapeutic interventions to promote optimal health outcomes.
- Collaborate with multidisciplinary teams to ensure comprehensive care.
Required Skills and Qualifications
- A minimum of two years' post-graduate employment in a clinical area directly applicable to the OPMHS and/or Huntington's Disease Service area.
- Demonstrated expertise in therapeutic interventions relevant to OPMHS and/or Huntington's Disease Service settings.
- Experience in liaison and consultation with other services and agencies to optimize service provision.
Benefits
This is a permanent, full-time position working 76 hours per fortnight. The successful applicant will be required to work collaboratively as part of a multidisciplinary team.
