Job Overview

The Huntington's Disease Service is a specialist team dedicated to supporting individuals and families affected by Huntington's Disease. Our team works closely with medical professionals, allied health, and community services to deliver integrated, evidence-based care.

Job Role

  • This role involves delivering specialized case management and clinical support to individuals and families impacted by Huntington's Disease.
  • You will coordinate care across community, inpatient, and outpatient settings, provide education and therapeutic interventions, and collaborate with health professionals and support services to ensure holistic, person-centred care.

Key Responsibilities

  1. Deliver high-quality, patient-centered care to individuals and families affected by Huntington's Disease.
  2. Coordinate care across multiple settings, including community, inpatient, and outpatient environments.
  3. Provide education and therapeutic interventions to promote optimal health outcomes.
  4. Collaborate with multidisciplinary teams to ensure comprehensive care.

Required Skills and Qualifications

  • A minimum of two years' post-graduate employment in a clinical area directly applicable to the OPMHS and/or Huntington's Disease Service area.
  • Demonstrated expertise in therapeutic interventions relevant to OPMHS and/or Huntington's Disease Service settings.
  • Experience in liaison and consultation with other services and agencies to optimize service provision.

Benefits

This is a permanent, full-time position working 76 hours per fortnight. The successful applicant will be required to work collaboratively as part of a multidisciplinary team.
