About This Opportunity

We are seeking a skilled professional to fill the role of Case Manager. The successful candidate will have a strong background in Nursing or Allied Health, with relevant registration, and previous experience in case management, particularly in community and aged care settings.

Key Responsibilities:

  • Develop personalized care plans and coordinate a multi-disciplinary team for the delivery of in-home therapy, services, and equipment.
  • Manage and coordinate the day-to-day delivery of care for clients, including individualized budgeting.
  • Advocate for clients and participate in case conferencing and scheduled clinical appointments as required.

Requirements:

To be successful in this role, you will require:

  • Tertiary qualifications in Nursing or Allied Health with relevant registration.
  • Previous case management experience, preferably in community and aged care settings.
  • Able to develop and maintain strong professional networks with internal and external stakeholders.
  • A current driver's license and comfort traveling throughout the region each day.
  • COVID-19 vaccination.
