Oncology Psychosocial Support Specialist

About the Role- Conduct comprehensive psychosocial assessments to identify patient needs
- Develop and implement individualized care plans with multidisciplinary teams
- Provide emotional support and guidance to patients, families, and carers
Key Responsibilities:
- Patient Support: Provide high-quality psychosocial care to oncology patients, including assessment and support.
- Discharge Planning: Develop and lead complex discharge plans with multidisciplinary teams to ensure smooth transitions for patients.
- Communication: Foster open communication with patients, families, and carers throughout the care process.
- Master's degree in Social Work or related field
- Current AASW registration or eligible for registration
- Strong skills in psychosocial assessments and discharge planning
- Fully vaccinated for COVID-19 (including booster)
